Google Ads for physiotherapy clinics: what to know before you spend

Ads can fill empty appointment slots within days, or quietly burn money. Here's when they make sense for a physio clinic, what clicks cost, how to set up a first campaign, and the rules that apply.

When ads make sense for a physio clinic

Ads buy you a place above the map results today. In Ontario, patients can book a physiotherapist directly without a referral, and many search the moment something hurts. That makes physio a good fit for search ads, as long as three things are true:

  • You have open appointment slots you could fill within a week or two.
  • You offer something people search for by name, such as back pain, sports injuries, rehab after surgery, concussion care or pelvic health.
  • Your Google profile and booking page are in good shape, so a click can turn into a booking.

If you're fully booked, skip ads for now. Put the money into your profile, reviews and service pages, and switch ads on when you add a practitioner or extend your hours.

What clicks and leads cost

Physiotherapy is one of the cheaper healthcare services to advertise. In LocaliQ's healthcare benchmarks, drawn from 3,542 US search campaigns between October 2024 and September 2025, physical therapy clicks cost a median of US$4.95. About 15% of clicks became a lead, and each lead cost about US$32.79. General dentistry, for comparison, came in at US$7.03 a click and US$84.77 a lead.

Those are US figures in US dollars, so use them as a rough guide. Your own account will tell you the real numbers within a few weeks. Here's the math with round numbers close to the benchmarks. It's an illustration, not a forecast.

StepExample
Monthly ad budget$1,000
Cost per click$5, so about 200 clicks
Clicks that become a call, form or online booking15%, so 30 enquiries
Enquiries that book an assessmentTwo in three, so 20 assessments
Ad spend per booked assessment$50

An assessment often leads to a plan of several visits, so the value of a new patient is the whole plan, not one appointment. Work it out from your own records: average visits per new patient times your fee per visit.

How to set up a first campaign

  1. Group your services into a few ad groups: general physiotherapy, sports injuries, and one or two services you want to grow.
  2. Target the neighbourhoods your patients actually come from, not the whole city.
  3. Use phrase and exact match keywords such as "physiotherapy near me" and "physio [neighbourhood]". Add negative keywords for searches that never book: jobs, salary, school, course, assistant program, free exercises.
  4. Send each ad to the matching service page with a Book button, never just the homepage.
  5. Add call, location and sitelink assets so the ad shows your number, your address and your main services.
  6. Make a booked appointment the main conversion. Jane can pass online booking activity to Google Analytics. Track calls and forms as separate conversions.

The rules that apply to your ads

The CPO's Advertising and Marketing Standard covers ads exactly as it covers your website. Claims must be truthful, accurate and verifiable. No guaranteed outcomes, nothing implying your care is better than other providers', and no wording that suggests a specialization you can't back up.

Google adds its own layer. Its personalized advertising policy restricts targeting based on health information, so don't build remarketing lists from people who visited a concussion or pelvic health page. Target the search, not the person.

Measure booked assessments, not clicks

The number that matters is ad spend per booked assessment, and then per patient who starts a plan of care. For the first month, read the search terms report every week and add negatives for anything irrelevant. Give a new campaign four to six weeks of data before you judge it, then cut keywords that spend without booking.

Our guide to appointment tracking shows how to count bookings by source while keeping patient information out of your reports.

Common mistakes

  • Broad keywords with no negatives, which pay for job seekers and students
  • Sending every ad to the homepage
  • Running ads while the schedule is already full
  • Counting clicks or calls as success without checking whether they booked
  • Letting Google's automated campaign types spend before booking tracking works
